This guide will show how to control a risk within QUOODA® through the Uncontrolled Risks page.
Step 1. Go to Main Menu > Risk > Uncontrolled Risks
Step 2. Review the risk
When you open the page, you will notice a list of the uncontrolled risks that are currently listed on the system. You can search and filter these if required. You can see more details on the risk by clicking on the double boxes next to the risk information.
TOP TIP: You can export a PDF of the risk as a ‘Risk Snapshot’. To do this, click the ‘three dots’ in the Risk ID column and click Preview or Download. If you do not see this column, please review the adding and removing columns section of this support article.
Step 3. Add an action
Click on ‘Click to create new risk action/evidence’ to add details to the risk. This is a mandatory field; action needs to be recorded before the risk can be controlled.
Within the ’Add new risk action’ screen you can:
- Add details of the action taken
- Set who it was actioned by
- Add an image or document by selecting the ‘Add Image’ or ‘Add Document’. This will open Windows explorer or Finder if using a MacOS, where you can choose the files to upload. Once you have chosen your file click the ‘Upload file button’.
You can also See Risk details directly from the Assessment such as Control Measure and Hazard if provided and Copy Risk details in full using the ‘Copy Full Details’ Button.
Step 4. Update the Risk Mitigation Status.
Depending on the stage you are at with the risk select the relevant status.
- On Hold
- Not Started
- In Progress
- Completed
- Abandoned
For this particular example, the action we have taken will close and control the risk so change the Risk Mitigation Status to ‘completed’, click the ‘controlled’ box and save.
You will now see that this has been controlled but is still accessible through the Risks > All Risks menu.
TOP TIPS:
- If you have saved a risk as ‘completed’ but not controlled the system logic will expect further action to be needed to control the risk. You therefore cannot click the controlled box once something is saved as ‘completed’. Change the status back to ‘In progress’ to indicate further work is being done. You will then be able to mark it as ‘completed’ and ‘controlled’. These status changes must be saved together to work
