Mandatory Occurrences are used as a way of tracking and reporting actual or potential structural failure of a building including the spread of fire and smoke through a building. Mandatory Occurrence notices must also be reported in the case of, or potential of a large number of deaths or serious injury.
Please note: if you do not see the Mandatory Occurrence Module, your role type will not have security access to it., Please ask your Super User to review your role security.
Step 1. Raise an Adverse Event
Before a Mandatory Occurrence Notice can be raised, you must first raise an Adverse Event. Instructions on how to do this can be found on the record a new adverse event page.
Step 2. Raising Mandatory Occurrence
Once the adverse event has been saved you can click on the Mandatory Occurrence tab within the adverse event record. Then click ‘Add New Mandatory Occurrence’ and the system will open a new tab.
Step 3: Adding Details
You can now add details to the Mandatory Occurrence:
- Type in the AR number or Display name and pick from the list to populate the property field.
- The incident number will be prepopulated.
- Please provide as much detail as possible to explain why you are raising this mandatory occurrence. This will help the Manager who reviews the occurrence evaluate it quickly.
Step 4. Adding Documents
You can add documents as supporting evidence.
- Click the documents tab and click the attach file button on the top right corner.
- Select 'Upload a file' to upload from something saved on your device.
- You can then choose the destination folder within QUOODA® to save the file to.
- Click 'Select Files'
- Choose the documents from your PC
- Click 'Upload Files'.
Step 5. Save
Once the information and documentation are complete, click ‘Save’ to create the Mandatory Occurrence record.
The Mandatory Occurrence module will load, and you will see your new Mandatory occurrence record within the table.
