Actions can be added to a risk to record something that has been done to progress the risk, or to set out something that needs to be done.
To follow the steps on this page you will need to have found the risk you are looking to add an action to.
Step 1. Go to actions
The actions section is indicated by the icon of an arrow in a target. There will be 2 numbers displayed in this section:
- The first is the number of actions taken (these are completed).
- The second is the number of actions required. This will be red if there are required actions outstanding.

Tap the actions section to proceed.
Step 2. Select Add a New Action
Step 3. Complete the fields
Complete as many fields as required, any fields marked with an asterisk are mandatory and the save button will not activate until they are completed.
- Action Name - A summary title for the action - If left blank, the action name will be populated with the first 25 characters of the Action Description.
- Action Description* - A detailed description of the requirements of that action.
- Priority* - Tap in the field and select the appropriate priority from the list.
- Action Status* - Tap in the field and select the appropriate status from the list. Actions given a status of ‘Completed’ will be shown in ‘Actions Taken’ after saving, all other statuses will show in ‘Action Required’.
- Attach Image - You can take a photo with the device camera or import one from your gallery to illustrate the issue that needs to be resolved or show the work that has been done. Images can be added in groups of 10, if you need to add more you can tap the camera icon and repeat this step as many times as required.
- Due Date* - Will default to the current date, tap on the field to load a calendar and select an appropriate date.
- Assign To* - Tap in the field and select the appropriate person to assign this task to from the list.
Step 4. Save
Once all mandatory fields have been completed the save button will activate. Tap it to save the new action.
- Actions given a status of ‘Completed’ will be shown in ‘Actions Taken’ after saving
- All other statuses will show in ‘Action Required’.
TOP TIP - As well as reviewing and updating actions from the risk, these will also be seen as tasks in the QUOODA® portal.
