This page will provide an overview of the data and options available within a risk record on the mobile app. To locate a risk and see this information please follow the steps on the finding a risk help page.
TOP TIP – There is a < arrow in the top left of the screen, you can press this at any point to go back to the previous screen.
The risk information is split across a number of areas and menu options, each of those areas is explained under the sub-headings below.
General Information
The risk number and rating are displayed in a coloured bar near the top of the screen. In this example, the bar is orange to signify a ‘Substantial’ risk rating (or Medium on some systems). You would see a red bar for ‘Intolerable’ (High) risks and a yellow bar for ‘Moderate’ (Low) risks.
Below that is a summary of key information, which includes:
- Status – This is the current status of the risk. This is the only part of this data that you can update via the app. To update the status, tap on the status area and select the appropriate option (as shown in the image below).
- Due – This is the due date of the risk and it is automatically set based on the date it was identified, and the rating of the risk.
- Priority – The priority level (if set)
- Type – Details the type of assessment the risk was generated from, e.g. Health and Safety Risks Assessment or Fire Risk Assessment
- Description – The details from the description field on the risk record
Control Risk
The Control Risk section is indicated by the icon of an exclamation mark in a circle with a tick.
Controlling a risk will complete and close it. You therefore need to record evidence that the risk has been correctly mitigated and specific steps need to be followed. We explain these in detail on control a risk help page. Please review this page for more information on this area.
Actions
The Actions section is indicated by the icon of an arrow in a target.
There will be 2 numbers displayed in this section:
- The first is the number of actions taken (these are completed).
- The second is the number of actions required. This will be red if there are required actions outstanding.

We cover how to add a new action and update an action required in their own help pages. Please review these pages for more information.
You can also view previous actions via this section. To do this tap on ‘Actions Taken’. There will be a box for each action that has been taken and you can tap on it to load more data. Within each action taken, you will be able to see the following information:
- Name
- Description
- Completed Date
- Assigned To
- Priority
- Action Status
You also have further menu items for Attachments and Comments, you can tap on these to load the details.
Additional Information
The Additional Information section is indicated by the icon of an exclamation mark in a square.
If you tap on this menu option, you will be able to view data from several key fields on the risk record:
- Location
- Topic
- Hazard
- Control Measure
- Guidance
- Related Legislation
Notes & Observations
The Notes & Observations section is indicated by the 3 lines icon. Where data exists, you will be able to see the number of available notes within the circle on the menu item.
Tap this menu item to load a summary of the available notes, you can tap again on any of the displayed notes to see the full detail.
Attachments
The Attachments section is indicated by the icon of mountains within a frame.
This area will contain any images or documents that have been uploaded to the risk itself or actions taken against the risk.
To access the attachments, tap on the menu option. A list of all the available attachments will be displayed, and you can click on them to load the file. If you are opening a document, you will be presented with a list of apps which can open that file type on your device. Select the app you wish to use (e.g. PDFs in Adobe Reader).
TOP TIP - To upload new attachments to risk, please update an action. You will only be able to attach images via the mobile app. If you need to attach another file type, you will need to do this via the QUOODA® Portal.
View Assessment
The View Assessment section is indicated by the icon of a document with an eye in the bottom corner.
If you tap this menu, the assessment to which this risk is linked will be downloaded in PDF format. Once the document has been retrieved, you will be presented with a list of apps which can open that file type on your device. Select the app you wish to use (e.g. PDFs in Adobe Reader) and the file will load in that app.
